Located right at the center of the face, the nose must be in perfect harmony with other facial features within the framework of symmetry and golden ratio rules. What is Rhinoplasty and In Which Situations Is It Performed

Nose aesthetics is a surgical operation performed to change the shape, size, or structure of the nose, or to eliminate functional disorders. Although it generally appears to be a method applied due to aesthetic concerns, it actually plays a vital role in eliminating breathing problems, deviations in the nasal bone, anatomical blockages leading to sinusitis, and damages occurring after trauma. Congenital anomalies or subsequent deformities can negatively affect both the physical health and social life of the individual. Thanks to this operation, removing the nasal hump, lifting or refining the nasal tip, reducing the size of the nostrils, and bringing the angles of the nose between the forehead and lips to ideal proportions are achieved.

Differences Between Open and Closed Rhinoplasty Techniques

When planning surgical intervention, two main techniques come to the fore depending on the degree of deformation in the patient's nose and the surgeon's preference: Open rhinoplasty and closed rhinoplasty. In the open nose aesthetics technique, a small incision is made in the small skin tissue called the columella between the nostrils, and the nasal skin is completely lifted. Since this method provides the surgeon with the opportunity to directly see the nasal cartilage and bone structure and make millimetric interventions, it is preferred especially in advanced deviations and revision (secondary) surgeries. In closed nose aesthetics, all incisions are made inside the nostrils; no suture marks remain on the outside. The closed method is known for the advantage of a faster recovery process and keeping edema and bruising at a minimum level, but the surgeon's angle of vision is more limited.

A New Era with Ultrasonic Rhinoplasty Piezo Technology

The Piezo method, namely ultrasonic nose aesthetics, which is one of the greatest innovations brought to aesthetic surgery by technology, has completely buried traditional bone-breaking methods into history. In this technology, by using ultra-high-speed vibrations created by sound waves, nasal bones are shaped millimetrically without causing any damage to surrounding tissues, blood vessels, and nerves. Since soft tissue damage does not occur, excessive bruising, swelling, and severe pain, which patients fear the most after surgery, are almost never experienced. Piezo surgery both offers a comfortable recovery period to the patient and maximizes the success rate of the operation by allowing the surgeon to work precisely on the bone like a sculptor.

Preoperative Planning and Simulation Studies

The secret of a successful rhinoplasty lies in a detailed examination and correct communication before the operation. The facial structure, skin thickness, cartilage quality, and bone structure of each person are completely different from one another; therefore, it is impossible to talk about a standard nose mold. While the complaints and expectations of the patient are listened to during the initial interview, the probable post-operative nose profile is designed on the computer screen with the help of advanced three-dimensional simulation devices and digital photo analysis. At this stage, a balance is established between the wishes of the patient and the anatomical limits that are surgically applicable. In addition, it is of critical importance for the patient to quit smoking at least two weeks before the operation and to stay away from blood thinners and herbal teas in order to reduce the risk of bleeding during the operation.

Postoperative Recovery Process and Things to Consider

The recovery process starting after the completion of the surgery is a period that requires at least as much patience and care as the operation itself. In the first days, silicone splints placed inside the nose to help breathing and special plaster or casts on the nasal dorsum protect the new shape of the nose. Applying a cold compress (ice) around the eyes for the first 48 hours following the surgery largely prevents the formation of edema. Patients need to sleep with double pillows with their heads elevated for the first few weeks, avoid heavy sports, pool and sea activities, direct exposure to sunlight, and most importantly, avoid using glasses for at least 3-6 months. The complete settling of the nose, total dissipation of edemas, and healing of tissues can usually take up to two years in thick-skinned patients, while it is completed in around one year in thin-skinned patients.

Nose Aesthetics in Men Preserving Masculine Lines

Rhinoplasty planning made for male patients involves much different dynamics and aesthetic criteria compared to female patients. While more upturned, curved, and elegant nasal dorsum lines are preferred in women; the nasal dorsum should come straight (without curves) and the angle between the nasal tip and the lip should be kept around 90-95 degrees in men so that the masculine expression is not lost. If the nasal tip is lifted excessively or the nose is reduced too much in male facial anatomy, an unnatural feminine appearance that completely contradicts the masculine facial features of the person may emerge. Therefore, the main goal in male rhinoplasty is to create a strong and natural-looking nasal structure that will strengthen the general masculine character of the face while solving breathing problems.
